I used IncFile.com and incorporated in Delaware. They have a free option, but I opted for the $199 option which included an operating agreement, EIN number (necessary for the bank account), and everything else I needed.
I work at a law firm and have attorneys at my disposal, and still I chose this path because it was easiest and most painless.

This depends on how many other founders there are, and whether there is any other staff as well.
Niche down. Pick an industry and develop websites specifically for that industry, so that you can dominate that niche.
For example, I build software, but I specialize in the divorce law/family attorney industry. This helps me convert more attorneys since my portfolio isn’t filled with other stuff they don’t care about.
I feel your pain. I was in a similar situation. I can offer some advice:
you need a really well trained and highly capable assistant to be your client relations officer: someone to deal with the clients and employees on your behalf that you rigorously train for 90 days and takes the client management burden off you.
you need to start automating the small repetitive tasks in your business so a monkey can do them and you’re not required for operations.
I did this for our company and it changed the morale of every executive. I’d be happy to discuss this with you and see if a similar plan could apply.
